While members receive a PIN upon enrolment or registration, they still need an ID card that certifies that they are a PhilHealth member.

A PhilHealth ID serves as proof that a member and their dependents can avail of the benefits and privileges provided by the government’s National Health Insurance Program. 

A PhilHealth ID card is a paper ID that PhilHealth members can use. The Department of Health defines a PhilHealth ID as the “unique and permanent number issued to any client upon enrolment/membership registration to the National Health Insurance Program of the Corporation,” usually for identification, eligibility verification, and utilization recording purposes.

Essentially, a PhilHealth ID serves as proof that a person is a PhilHealth member and allows them to avail of PhilHealth’s health insurance and benefits. PhilHealth Members can and are encouraged to claim their PhilHealth ID at any PhilHealth Local Health Insurance Office (LHIO) free of charge. 

Who can apply for and claim a PhilHealth ID?

How to become a registered PhilHealth member:

For Members of the Formal Economy 

Formal Economy Members include:

  • Government employees
  • Private employees
  • Workers that render services, whether in government or private offices, such as project-based contractors.
  • Owners of Micro Enterprises
  • Owners of Small, Medium, and Large Enterprises
  • Household Help as defined in Republic Act 10361 or “Kasambahay Law.”
  • Family Drivers

For Newly Hired And Existing Employees Without PIN Yet as of August 2022

Step 1: Prepare two copies of the PhilHealth Member Registration Form (PMRF).

The form is downloadable online for you to print out. Make sure to fill out both printed copies of the registration form, especially the boxes for your personal information.

Step 2: Submit the filled-out PMRF to the HR Department of the employer.

The employer is responsible for reaching out to PhilHealth to submit the employee’s filled-out PMRF and other documents, such as valid IDs and pictures. 

Step 3: Wait for the employer to provide the Member Data Record and PhilHealth ID card.

Once you receive the MDR and PhilHealth ID card, you will settle your premium contribution based on the new premium contribution rate.

After deducting half of the premium requirement from your monthly salary, the employer remits the total premiums to PhilHealth.

For Newly Hired And Existing Employees With PIN as of August 2022

Step 1: Report your PIN to your employer for them to indicate the same in their ER2

The ER2 indicates the names of newly hired employees. It’s the employer’s responsibility to submit these records to PhilHealth.

For Members of the Informal Economy 

Informal Economy Members include:

  • Migrant Workers (documented or undocumented Filipinos engaged in a remunerated activity in another country of which they are not citizens.)
  • Informal Sector (includes street hawkers, market vendors, pedicab and tricycle drivers, small construction workers, and home-based industries and services.)
  • Self-Earning Individuals
  • Filipinos With Dual Citizenship (Filipinos who are also citizens of other countries.)
  • Naturalized Filipino Citizens (those who have become Filipino citizens through naturalization as governed by Commonwealth Act No. 473 or the Revised Naturalization Law.)
  • Citizens of other countries working or residing in the Philippines (foreign citizens with valid working permits or Alien Certificate of Registrations ACRs working or residing in the Philippines)

Step 1: Visit the PhilHealth portal to locate the nearest PhilHealth Local Health Insurance Office or PhilHealth express outlets. 

You can locate the PhilHealth Local Health Insurance Offices and PhilHealth express outlets within your area using the PhilHealth portal.

Step 2: Prepare two copies of the PhilHealth Member Registration Form (PMRF).

The form is downloadable online for you to print out. Make sure to fill out both printed copies of the registration form, especially the boxes for your personal information.

Step 3: Prepare two valid government IDs, your (NSO) birth certificate, and two 1×1 recent passport photos.

Step 4: Submit the documents to your chosen PhilHealth Local Health Insurance Office or PhilHealth express outlet. 

Submit your filled-out PhilHealth Member Registration Form and IDs to the officer-in-charge at your chosen PhilHealth Local Health Insurance Office or PhilHealth express outlet. 

Step 5: Wait for your documents to be processed and for your Member Data Record (MDR) and PhilHealth ID Card.

Step 6: Pay the necessary premium contribution using your PhilHealth ID number.

Regular ID (free for all active PhilHealth members)

How to get your ID if you don’t receive it through your company’s HR department:

Step 1: Visit any PhilHealth Express branch. 

Step 2: Bring two valid government IDs and the PhilHealth Insurance number assigned to you upon applying for membership.

Step 3: Go to the officer-in-charge and ask for a “Request Form.”

Step 4: Accomplish the Request Form thoroughly and submit it to the officer-in-charge.

Step 5: Double-check each section of the Form to ensure all the information you put is correct.

Digitized PhilHealth Insurance ID Card (optional for employed members)

How to get your ID if you don’t receive it through your company’s HR department:

Step 1: Refer to the step-by-step process previously mentioned for the regular PhilHealth ID but this time, check the option for the Digitized Insurance ID in the Request Form. 

Step 2: State the names of your dependents. Your insurance will also protect your dependents. Adding dependents will require you to submit two of your dependents’ government valid IDs on their behalf.

Step 3: Pay a standard fee of 90 PHP. You will have to pay a fee of 358 PHP for applications with dependents.

What’s the difference between the two IDs?

The Regular PhilHealth ID is applicable in availing of PhilHealth Inpatient and Outpatient benefits at accredited HCIs.

The same goes for the Digitized PhilHealth Insurance ID Card. What makes the latter better is that it can also serve as a valid government ID for other transactions, including banking, monetary exchange, and identity verification.

However, the issuance of the digitized ID to voluntary members is currently on hold.
